Registration

All requests for Behavioral Health Services will be submitted Direct Data Entry (DDE) at https://careconnectionwv.kepro.com

No request for services will be faxed or authorized telephonically.

All providers of WV CHIP services will have to register for this service whether previously registered or not. Please utilize WV CHIP in your name, for example: Organization name Prestera WV CHIP and the NPI you are registered with for these Services.

Registering WV Children's Health Insurance Program

  1. First, fill out the “Provider Registration” form for your agency specifying the practice name, mailing address, phone/fax numbers, etc. where indicated. Omit the “Provider ID” field. This will be assigned. 
  2. In the next section, specify a "Data Contact" (an individual of your agency who will be the primary point of contact (electronic/other) with KEPRO and will be authorized to establish/authorize web users for your agency). Fill out the data contact's address (which could be different from your corporate address) specifying the data contact's email address (important for electronic communications), and include the data contact's signature.  Finally, have the appropriate level of leadership authorize the specified data contact via their signature at the bottom of the provider registration form (e.g. this could be your agency's CEO, Executive Director, Director of your Behavioral dept., Owner of your Agency/Practice), to authorize the data contact responsibilities - maintaining web users (requesting activation/deactivation) and primary point of contact for electronic communications with KEPRO.    
  3. Then make/print copies of the “Web User Request” form and fill out one for each individual who need access to the KEPRO secured web site, as a "User ID" will be created for each web user form received based on the user's name and identifying information. The web user must sign and date on the "User Signature" line, and the data contact must sign and date on the "Data Contact's Signature" line effectively authorizing the web user. When/if new web users are needed in the future, simply send additional web user forms only, and those new users will be added to your agency.
